This patient has a history of allergic fungal sinusitis. Endoscopic sinus surgery was performed at another institution approximately 2 years ago. Despite interim aggressive medical therapy with antibiotics, oral and topical steroids and allergy therapy, the patient has had return of severe sinonasal symptoms. On evaluation in the office, nasal endoscopy revealed polyps and infected sinus secretions. A CT scan confirmed fluid and inflamed, thickened tissue throughout the paranasal sinuses. Revision endoscopic sinus surgery with image guidance was discussed and the patient elected to proceed with the surgery. The procedure went without complications and the patient had minimal post-operative discomfort. Nasal endoscopy performed approximately 3 months after surgery was recorded and is seen here. The patient had significant reduction in sinonasal symptoms and was very pleased with the outcome.
